I apply and qualify for a Corporate Membership (or an extension of my current Corporate Membership) for the PTOS since I am not employed by the PTOS and wish to order at least five (5) copies of the Journal.
$240/1 yr. for Domestic orders.
$280/1 yr. for Canadian orders.
$360/1 yr. for Foreign orders.
Number of Issues Requested*
*The above price list will be applied to orders of five (5) issues. Larger orders will be priced accordingly, i.e. an order for six (6) issues would be priced at $300 for a Domestic order, $240 for the first five (5) copies and $60 for the sixth copy, and an order of ten (10) issues would be priced at $480 for a Domestic order.
